هنا ان شاء الله هنحط كل حاجه عن امتحان عملي الحالات عشان كلنا نفيد ونستفيد
ياريت تطمنونا عليكم وعلى الامتحان وعملتو ايه فيه ويارب تكونوا مبسوطين وجاوبتوا حلو

انا امتحنت النهاردة ولله الحمد

الناس بتتواجد على حوالي 8 ونص كده
الكل بيتجمع في الدور الرابع في مستشفى الجراحة
بتسجل اسمك وتدفع 70 جنيه ويقولولك متدفعش جوه ومع ذلك العيان بيقعد يقول لك عايز فلوس...ممكن تديله وممكن لا انت ومزاجك
وبعد كده بييجي الدكتور المراغي وبيوزع الناس عشوائي ...يعني التوزيع مش بالاسماء

انا دخلت الوحدة السادسة...مفيش 6أ ولا 6ب كلها اسمها 6 جراحة

دخلت على حالتين شورت وامتحني فيهم دكتور واحد
بيخليك تاخد هيستوري سريع وفحص سريع في 10 دقائق لكل حالة وبعد كده بيسألك عن الحالتين مرة واحدة
والامتحان كان تقريبا شفوي
الحالتين كانوا subcutaneous lipoma in back&primary varicocoele
الاسئلة كانت عادية
عرفت منين انها التشخيص ده؟؟؟ايه انواع الليبوما؟؟؟ايه اخطرهم؟؟؟ايه الفرق بينها وبين السيباشوس سيست؟؟؟

عرفت منين انها بريماري وايه الفرق بينها وبين السكندري؟؟؟ايه سبب كل واحدة؟؟؟ايه الانفستجيشنز فيها؟؟ايه الفينز المشتركة فيها (فيه واحد غير البامبيني فورم بليكسس وهو الكريماستريك فيين)؟؟ايه الكومبليكيشنز؟؟؟ايه الانديكيشنز للجراحة؟؟؟لو العيان قالك هخلف بعد العملية هتقوله انه هيخلف بنسبة كام؟؟؟(هي نسبة قليلة طبعا لكن انا معرفتهاش بالضبط وقلتله حوالي 20% فرضي بيها على اساس انها قليلة وقال انه فيه نسبة ارجعلها)..العيان يعمل تحليل للسبرمز بعد اد ايه من العملية؟؟(3 شهور)

الدكتور بدأ معايا من غير ما اكمل الشيت وكان طيب جدا ...قلتله لسه ماخدتش غير البرسونال هيستوري والكومبلينت قاللي مش مهم...وكان الحوار بيننا كويس فيما يتعلق بالحالة وخلاني اشرحلة التيستات على العيان ...وبس كده

شدوا حيلك يا شبااااااااب
ركعتين قيام ليل في الثلث الأخير مع دعوتين من الحاجة وصلاة الصبح جماعة في المسجد وانت هتكون في ذمة الله

my long case was simple multi nodular goiter.
i just said the personal history and complaint.
he didnt complete the sheet . he asked me about
then shit happens and doctors make a noise it was very loud and they were objeting on some thing . and my short case started(doctor sayed rashad)
1)1ry left sided varicocele not complicated.< asked y on left .y 1ry ,causes of 2ry and what is the retroperotenial tumor may make 2ry varcocele.i dont knew it.
2) oblque inguinal hernia complicated not reducible. he asked me y indirect , what is the inguinal region !!!!!! i said above ing ligament and i said the anatomy of it . he said nooooo , then afight started and the doctor told me i will give u agood mark dont worry and goo now !!!!!!

there were : case of parotid!!!

breast lump.

hi every one,

i hope all be relax not stressed & take it easy.it will pass well Isa.

i examined at 7 department.
my long case was splenomegly for differntial diagnosis ( some say it was chronic hemolytic ananemia) (don't loss ur concentration when the patient asked 4 money )& then sinuor called me rapidlly ( i not yet finished my examination) , the dr listened 4 personal history & complain only then asked me ( wat causes of pain in left hypochondrium? common causes of splenomegly? wat the chrchr of spleen enlargment? wat the types of colic?)

my short cases were varicocele & ingunial hernia . the dr. tested me on the hernia case( how 2 examine the groin? how 2 examine the testis? but take care when u examined the testis u must support scrotum posteriorly by the left hand....& then asked about the sebecous cyst which was present at scrotum? ) then shift to the neck how u exmine it on same patient!! then asked for pustile swelling in the postior triangle?( the neck examination was instead of the varicocele) the dr. choose by his mind..

وكمان انت بتعرف تعمل اكسامينيشن ولا لأ
وبتعمل كل حاجة ولا لأ
بس كان مش بيبين هو مبسوط ولا متضايق ولا انا صح ولا غلط
ودي حاجة بتنرفزني جدا

- what r the 4 most important symptoms of thyrotoxicosis
palpitation, wt loss inspite polyphagia, easy fatigue , 4th i dont know but i mentiond all symptoms i know ' but dr want the most imp." and he stop me on most of the symptoms to analyze other possibility of this symptom other than thyrotoxicosis
the most common mentioned other possibility is metastasis , and other causes of dyspnea other than the positional dyspnea ???
and so on

my exam was in the 6 unit, i don't know the names of doctors
long case was varicose vein , i choose thes case cause i was the last one.
doctor ask so many question may be for about 20 minutes.
what is ur diagnosis?? why it's primary?? eldoctor aly 3amlty examination abdomen or not ana makontsh 3amlt, so he said i can consider ur diagnosis false as u can't exclude except after abdomen examination.
type of thrombosis, symptom of DVT , risk group of DVT, complication of DVT, mechanism of this complication, what is first thing to do in patient with phlagmasia?? doctor want to hear fachiotomy , types of pulmonary empolism?, the most dangerous complication for DVT , eldoctor was meaning DEATH, lw big pulmonary embolism,
managment of DVT, types of anticoagulant, the control time the antidote, surgical ttt of DVT, site of the filter , site of saphenous opening?, difference between doppler and duplex, indication of venography, special tests of varicose veins, pertsh test not enough to give idea about deep system we need for duplex.

SHORT CASES was varicocele and incisional hernia, questione were so simple,
types of surgery in varicocele? what we ligate is pampiniform and cremastric vein, important points in personal history of this patient,
incisonal hernia may be paralytic type ttt is hernioplasty , type of mesh

the long case was (varicose veins?) the docotr asked about diagnosis ,full one ,,and asked about dvt and the types of thrombus and how to evaluate acute dvt and the right answer was FDPs level ..he asked about the typre of thrombus ..the delayed healing of venous ulcer dt tissue hypoxia and how to treat (by fascial ligation)after conservative ttt of vv


the shoft cases were (parotid swelling)&?(varicocyle) about parotid mention the defrential diagnosis of swelling in this place and the facial nerve infiltration and superficial temporal artey ...and the contents of the parotid gland

the varicose ....most important 1ry or 2ry and why ...and don't forget u should eamin abdomin for hypernephroma for 2ry case

my long case was 1ry toxic thyroid goiter ( graves )
the doctor asked me how i reached this diagnosis from history,general and local examination....how to detect eye signs and he had some notes about the technique
what's true and false exophthalmos ? what's muller's muscle and its nerve supply
give full account on pulse ? what's water hammer pulse ?
how to diagnose retrosternal goiter,types of it and how to prepare the patient with toxic RSG for operation ?

my 1st short case......lt sided 1ry varicocele uncopmlicated
i was asked how i reached the diagnosis...symptoms..signs...complications...and don't say subfertility...say oligospermia !! .......treatment...and some operative details about varicocelectomy

my 2nd short case was implantaion dermoid cyst (3am gaber :D )
don't say this diagnosis....say it's a swelling for DD and the patient has a history of trauma suggesting implantation dermoid cyst
the Dr asked about types of dermoid cyst and treatment of my case

i paid 70 L.E........ 25 L.E for the short case....10 L.E for each short case

don't worry..the impression about all exams was good and the majority of my friends were happy

Al7amdolellah it was fine. Everyone is relatively cooperative:

We were allowed to choose one of the 2 short cases and our long case. The available ones were:

- Long case: spleenomegally, thyroid (don't know the case exactly), varicose veins, ischemia.

- Short: subfascial lipoma in the right side of the back (we all had to take it), varicocele, hydrocele, inguinal hernia.

Questions in general were around complications and types, some odd questions were:

- Lipoma (the patient said its submuscular BUT IT IS NOT! TAKE CARE PLEASE! It is SUBFASCIAL)
What are the types of classification ?! He wanted to hear according to site bla bla bla and according to pathophysiology.
Which back muscle is the lipoma attached to and how to put it in action?! I couldn't answer that really..

- Varicocele: What are the approaches in treatment? He wasn't satisfied when I just told him the names of the operations.

-Varicose veins: theories of ulcer formation (name and details!), cause of itching and pigementation (hemosidrin), course of the long and short saphenous.

الحمد لله الحالات كانت كويسة ,, النواب رعبونا في الاول و قالوا ان الحالات كوليسيستايتس و سيرفايكال ليمفادينوبثي ,, !!!!
بس الحمد لله الوضع اختلف لما دخلنا
الحالات بتاعة اللونج اللي شفتها كانت كالتالي

دة اللي امتحنت عليه و كانت الاسئلة عادية جدا زي اللي فوق و سأل على على علاج الثرومبوفليبايتس,, بس الدكتور كان ماسك فيس اوي ,,, بس طيب

باقي الناس اسألتهم متكررة فوق ,, مافيش حاجة غريبة اوي غير ان في دكتور سأل في الفاريكوس فين ,,, انواع الستوكينج ايه ,, و المفروض العيان هو اللي ينزل يشتريه و لا يبعت حد
و يستعمله بالليل و لا بالنهار ,,,

مش عارفة الانواع ,,, بس لازم العيان يجيبه لنفسه عشان يبقى مناسب لحجمه

دخلت علي حالتين شورت الاول الاولي كانت sc lipoma اللي شوفناه عند دكتور مطري
والتانيه كانت bilateral 1ry varicocele
عملت اكزامينيشن وخلصت ورجعت للاوضه اللي كانو مجمعينا فيها لغايه لما الدكتور يمتحني

الاسئله كانت سهله قوي ..سالني ال diagnosis ,why lipoma ,types of lipoma .وايه هي اخطر انواع ال lipoma
وهل فيه تقسيمه لل lipoma علي حسب consistency
وسالني ليه قلت varicocele وليه 1ry وليه اكتر علي الناحيه الشمال


الحاله اللونج كانت chronic venous ulcer as complication of dvt العيان ده شفناه عند دكتور مطري
وسالني ليه قلت عليها venous ulcer
ايه هي ال perforators الاناتومي بتاعهم يعني ...وازاي افرق بين 1ry.2ry varicose vein '

1- 3an elfractures wel diagnosis
2-s2alny 3an elinjury of soft tissue ( hematoma)
3- site of sweeling welbluish discolorition of hematoma ( around ankle "dependent")
4-if enlarged cause eh ( compartmental s) ttt ( fascitomy)
5- if small how to tt ( consevative "leg elevation , bed rest , he said aspirin as i think not sure ")
6- why is it imp. to differntiate between it and DVT ( we don't give heparin as it well cause more extravasation of blood )
7- it can lead to DVT ithink because of immobilization
as u see it wasn't conventional but the dr was nice but he was in hurry
at last he took me to the pt and asked me to describe ulcer as usual
short cases
1- rt sided oblique ing hernia not complicated same qs as usual ( why hernia , complications , inv ( none it's a clinical diagnosis ) ttt hernioplasty is the standard method now
2- lt sided 1ry varicocele as usual also ( why varicocele ,why 1ry and so on the only new qs was in subinguinal approach comp.is we can injury sympathatic chain around testis and coz testicular atrophy and laparscopic varicocelectomy is for bilateral cases or if ass. with hernia ( actually both qs i have red before 7ad kan katephom wkatep egapathom bgad rabena ykremo aw ykremha yarap ) adv. of palomo
i don't know the name of the dr of the long case and the short was avery nice dr. his name is "mohamed elshaf3y"
it's agood unit with good dr. u don't get to choose cases but it was easy anyway
i paid 4o L.E. for the long and 20 L.E. for one case and 10 for theother

the long case was v.v the examiner was dr.Nabil El said ,he asked me what is your full diagnosis ? (left sided 1ry v.v affecting long and short saphenous system incompitant either SFJ nor perforators comptant deep system without complication )
the most important word in yr diagnosis ? ... ( 1ry )
what is the causes of 2ry ? ..... ( DVT )
what is the most common site ?..... ( calf veins )
what is the ttt ?....( trendilberg's operation )
short cases were hydrocele / dermoid cyst
and the questions were easy

salam 3alikoum.. my exam was in unit 7.. first it started was short cases.. i was examined by dr.hamed he seems malignant but they say he gives good marks. the pts were really coopreative... i paid 20 each bmazagy and they didn't ask for more... 1 was 1ry varicocele, not complicated, why 1ry..., do scrotal neck test.. (don't forget to ask the pt to cough).. and do scrotal elevation test.. what are causes of 2ry varicocele... what is the %. of v..with hypernephroma (don't know and he didn't say)..what are the bounderies of renal angle.., 2nd was subcat. lipoma on the right side...inspect this swelling , examine the consistancy(flacctuation test).. examine the mobility.. tell the pt o push the wall.. what ms is ti realated to, serr. ant.. what are the complications...
long case, a ch. pt with 1ry v.v , not complicated, incomptent saphenofemoral on both sides, deep system patent..exam was in doctor office and i didn't do any thing on the pt, dr name was hossam and he was very nice , he gives very good marks, the qs were....what is you case... what do we mean by v.v... where elsecan we see v.v, what are the compl. of v.v... if u fasced a pt bleeding from v.v in emergency room ,what should u do... (elevation of the limb and compression by bandege- robat shash ya3ni).. what operation should u do to this pts, when to inject v.v, what is the materiak we inject.... and i was done...
about the other long cases in the unit there were more than 4 cases abdomen , some of them were acute cases.. my friend faced a case of acute adhesive I.O.. there was a case of c.c.c, other were chronic pts ,splenomegaly in a ch. heamolytic anemia pt., grave's, simple nodular goiter.... in the short ex. there was a case of epigastric hernia...and i.s hernia.... good luck everyone rabna ma3akom...
